Job summary

Family Pillars Homecare, LLC is seeking dedicated, clinically sound RNs and LPNs to provide skilled nursing care to patients in their homes on a flexible per diem basis across Florida.

Responsibilities include intermittent visits, shift-based care, wound care, IV administration, medication management, and care plan coordination with physicians and interdisciplinary teams. Florida license, current BLS, Level 2 background screening, and TB screening are required.

Qualifications

• Active Florida RN or LPN license (Compact License accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical (within past 6 months).
• Experience in Medicare-certified Home Health strongly preferred.

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation for RN).
• Uphold infection control standards and agency protocols.
