Vitality Hospice & Palliative Care is seeking a Per Diem Registered Nurse to join our clinician-owned team. You will provide hospice visits with admissions, assessments, and ongoing evaluations in patient homes across New Jersey, partnering with the Interdisciplinary Team.

This role requires active PA RN license, 1 year RN experience, travel to assigned areas, and a valid driver’s license. The schedule is every other Saturday and Sunday, with pay between $50.00 and $60.00 per hour, plus benefits

Qualificações

• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate with the hospice physician and Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.

Responsibilities

As a Hospice Visit Nurse, you will:
