Vitality Hospice is seeking a Per Visit Registered Nurse to join our clinician-owned hospice team. This role focuses on delivering high-quality bedside nursing care in patient homes and facilitating coordinated plans of care with an interdisciplinary team.

Responsibilities include admissions, initial assessments, ongoing evaluations, and education for patients and caregivers. Travel within assigned service areas is required; strong communication is essential for successful care delivery.

Qualifications

• Active PA RN license.
• Minimum 1 year RN experience.
• Willingness to travel within assigned service areas.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Strong communication and interpersonal skills.

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
