Vitality Hospice & Palliative Care is seeking a Per Diem Registered Nurse to provide expert nursing care in patients' homes across New Jersey. The role includes admissions, assessments, care planning, and collaboration with the Interdisciplinary Team to ensure optimal symptom management and quality of life for patients and families.

The Per Diem position supports travel across service areas, with a focus on compassionate care, patient safety, and thorough documentation.

Qualifications

• Active PA Registered Nurse (RN) license
• Minimum 1 year of RN experience
• Willingness to travel within assigned service areas
• Valid driver’s license, reliable transportation, and active auto insurance
• Strong communication and interpersonal skills

Responsibilities

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.

Responsibilities

As a Hospice Visit Nurse, you will:

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
