Vitality Hospice is seeking a Per Diem Registered Nurse to join our team and provide compassionate home-based nursing care. This role involves admissions, assessments, and ongoing evaluations during patient visits, with a commitment to comfort and quality of life on the end-of-life journey.

The position is part-time/per diem with scheduling every other Saturday and Sunday. Travel within service areas is required; compensation ranges from $50.00 to $60.00 per hour, with benefits including 401(k),

Qualifications

• Active PA RN license.
• Minimum 1 year of RN experience.
• Willingness to travel within assigned service areas.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate with hospice physician and interdisciplinary team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care.

Responsibilities

As a Hospice Visit Nurse, you will:

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.

• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
