Job summary

Vitality Hospice & Palliative Care is seeking a Per Diem Registered Nurse to join our clinician-owned team. You will provide in-home nursing visits, admissions, and ongoing assessments to relieve symptom burden and support quality of life.

Collaborate with the hospice physician and interdisciplinary team to implement care plans, supervise aides, and educate patients and families on safety and comfort. Travel within assigned service areas is required.

Qualifications

• Active PA Registered Nurse (RN) license.
• Minimum 1 year of RN experience.
• Willingness to travel within assigned service areas.
• Valid driver's license with reliable transportation and active auto insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Provide professional nursing care, including admissions, initial assessments, and ongoing evaluations.
• Develop and update individualized plans of care based on patient needs.
• Collaborate with hospice physician and Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement to elevate quality of care across the agency.

As a Hospice Visit Nurse, you will:

• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.

• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
