Job summary

Vitality Hospice & Palliative Care is seeking a Per Diem Registered Nurse to join our patient-centered team. This role delivers compassionate in-home nursing care, admissions, assessments, and ongoing evaluations in the community.

You will collaborate with an interdisciplinary team, educate families, and support home health aides while maintaining high standards of documentation and clinical excellence. Travel within service areas is required, with every other Saturday shifts.

Qualifications

• Active PA RN license required.
• Minimum 1 year of RN experience.
• Willingness to travel within assigned service areas.
• Valid driver’s license, reliable transportation and active auto insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Provide professional nursing care, including admissions, initial assessments, and ongoing evaluations.
• Develop, implement, and update individualized care plans based on patient needs.
• Collaborate with hospice physician and Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety and understanding.
• Maintain documentation, compliance and clinical excellence.
• Contribute to quality improvement across the agency.

Responsibilities

As a Hospice Visit Nurse, you will:

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
