Job summary

Vitality Hospice & Palliative Care is seeking a Per Visit Registered Nurse to join our team. This role focuses on compassionate end-of-life care with an interdisciplinary team to support patients and families in their homes.

The Per Visit RN works on Saturdays and Sundays on an every-other-weekend schedule and travels to patient residences, delivering assessments, care plans, and education while maintaining documentation and quality standards.

Qualifications

• Active PA Registered Nurse (RN) license.
• Minimum 1 year of RN experience.
• Willingness to travel within assigned service areas.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
