Vitality Hospice is seeking a Per Diem RN to join our clinician-owned team, providing compassionate home-based nursing care across assigned areas.

The role emphasizes admissions, assessments, and ongoing evaluations, with collaboration to ensure quality interventions. Travel is required within service areas; driver’s license and reliable transportation are essential.

Qualifications

• Active PA Registered Nurse (RN) license
• Minimum 1 year of RN experience
• Willingness to travel within assigned service areas
• Valid driver's license and reliable transportation
• Excellent communication and interpersonal skills

Responsibilities

• Provide professional nursing care, including admissions, initial assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care.
• Collaborate with hospice physician and Interdisciplinary Team for interventions.
• Oversee and support home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety and comfort.
• Maintain documentation, compliance, and clinical excellence.
• Contribute to quality improvement efforts across the agency.

Responsibilities

As a Hospice Visit Nurse, you will:

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
