Job summary

Vitality Hospice & Palliative Care is seeking a compassionate Per Diem Registered Nurse to join our clinician-owned team. You’ll provide in-home nursing visits, conduct admissions, initial assessments, and ongoing care evaluations in patients' residences, collaborating with an interdisciplinary team to manage symptoms and improve quality of life.

Responsibilities

• Provide professional nursing care including admissions, initial assessments, and ongoing evaluations.
• Develop and update individualized care plans based on patient needs.
• Collaborate with hospice physician and Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate quality of care.

Responsibilities

As a Hospice Visit Nurse, you will:

• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
• Oversee and support the work of home health aides, social workers, and chaplains.
• Educate patients and caregivers to promote safety, comfort, and understanding.
• Maintain high standards of documentation, compliance, and clinical excellence.
• Contribute to continuous improvement efforts to elevate the quality of care across the agency.
