Job summary

Transitions Hospice is seeking an RN Case Manager to coordinate care for patients and families in the Great Lakes region. You will anchor clinical decisions, guide the interdisciplinary team, and provide compassionate, hands-on care in home settings.

The role offers Mon-Fri hours with on-call rotation, competitive hourly pay of $40–$46, and comprehensive benefits, including medical/dental/vision, disability, a 401(k), and company-provided equipment. Active Michigan RN licensure required.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One year Hospice, home health, or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy

Schedule: Mon-Friday (8am -5pm) with back-up on call requirements

Pay Range: $40.00 - $46.00 am hour
