Job summary

Transitions Hospice - Illinois is seeking an RN to provide compassionate, clinical care in Champaign and surrounding counties. You will perform assessments, develop care plans with physicians, administer medications, document care, and support families.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end‑of‑life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs
• Build and adjust personalized care plans in collaboration with physicians and care teams
• Administer medications and treatments per physician orders
• Keep clinical documentation accurate, timely, and meaningful
• Serve as the communication bridge between families, physicians, and community health partners
• Educate patients and caregivers so they feel confident and supported
• Participate in on‑call rotation per team policy

Salary Range

$70,000 - $90,000 a year
