Job summary

Transitions Hospice LLC in Naperville, IL is seeking an experienced RN Case Manager to coordinate care for patients at home. The role anchors the clinical team, develops care plans, administers physician-ordered treatments, and ensures accurate documentation while supporting families.

The position offers a competitive annual salary ($85,000–$95,000), full benefits, and a rotating schedule with 4-day and 3-day weeks including weekends.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.
