Job summary

Transitions Hospice is seeking an experienced RN for an Extended Care Team (ECT) Nurse role covering Champaign and surrounding areas. Expect varied shifts with an 8 AM–8 PM schedule, rotating 4-day and 3-day weeks and weekend coverage in rotation.

TX: Hospice RN with active license, hospital/home health background preferred, capable of fast, accurate assessments and effective communication with physicians. Competitive pay and full benefits offered.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Flexibility and confidence working across varied patient assignments.
• Strong assessment skills and sound clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding accordingly.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support seamless care transitions.
• Ensuring equipment and necessary services are arranged and available for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.

What You'll Do
