Job summary

Trinity Health At Home – Grand Rapids, MI is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes, develop care plans, administer medications and educate families.

You will work with patients transitioning from hospital to home, enjoy autonomy, flexible schedules, and meaningful 1:1 care. Competitive pay, benefits starting Day One, and a $15K sign-on bonus are offered.

Qualifications

• Graduate of an approved Nursing education program; BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Minimum of one year professional RN experience.
• Home health or Hospice experience is a plus.

MinimumQualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred
• Home health or Hospice experience is a plus
• Must havecurrentDriver’s license and reliable transportation.
