Trinity Health At Home – Grand Rapids is seeking a Part-time Home Health Registered Nurse (RN) to deliver skilled nursing care in patients’ homes, develop individualized care plans, administer medications, and educate patients and families to support recovery and independence. Qualifications

• Graduate of an approved Nursing education program; BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred.
• Home health or Hospice experience is a plus.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes.
