Trinity Health At Home – Muskegon, MI is seeking a Homecare RN to deliver primary nursing care in patients' homes, develop individualized care plans with the Population Health Case Manager, and ensure outstanding patient outcomes and satisfaction.

You will conduct admission assessments, implement plan of care, provide therapeutic education, monitor changes, document accurately, and mentor peers to uphold quality and compliance with regulatory standards.

Qualificações

• Graduate of an accredited nursing program; BSN preferred.
• Current RN licensure in the state of practice.
• Minimum one year of professional nursing experience; home health experience preferred.
• Strong computer and technology skills.
• Current driver’s license and reliable transportation.

Responsabilidades

• Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.

Position Purpose
