Job summary

Trinity Health At Home – Grand Rapids (Michigan) is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes. You will create individualized care plans, administer medications, educate patients and families, and support recovery and independence.

Whether experienced in home health or transitioning from hospital, you can practice at the top of your license with autonomy and meaningful 1:1 patient relationships.

Qualifications

• Graduate of an approved nursing education program; BSN preferred.
• Current Michigan RN license.
• Minimum one year of professional RN experience.
• Home health or Hospice experience a plus.
• Valid driver's license and reliable transportation.

Minimum Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred.
• Home health or Hospice experience is a plus.
• Must have current Driver’s license and reliable transportation.
