Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes and build individualized care plans. You’ll educate patients and families, administer medications, and support recovery and independence in a 1:1 setting.

Qualifications

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in Michigan.
• Minimum of one year professional RN experience.
• Medical-Surgical experience preferred.
• Home health or Hospice experience is a plus.
• Strong computer and technology skills required.
• Must have a current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients’ homes and create individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence with 1:1 patient relationships.
• Practice at the top of your license with autonomy and flexibility.

MinimumQualifications

• Graduate of an approved Nursing education program, BSN preferred.

• Current Registered Nurse licensure in the State of Michigan.

• Must have a minimum of one (1) year professional registered nursing experience.
