Job summary

Trinity Health At Home Grand Rapids is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes, develop individualized care plans, administer medications and educate patients and families to support recovery and independence.

Whether you are seasoned in home health or transitioning from hospital, this role offers autonomy, meaningful 1:1 patient relationships and a path for professional growth within a faith-based, not-for-profit organization.

Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one year professional registered nursing experience.
• Home health or Hospice experience is a plus.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient status.
• Educate patients and families to support recovery and independence.
• Provide 1:1 patient care with autonomy and compassionate nursing support.

MinimumQualifications
