Trinity Health At Home – Grand Rapids is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes and build individualized care plans. You will administer medications, educate patients and families, and support recovery and independence in a 1:1 setting.

Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Minimum of one year professional RN experience.
• Medical Surgical Experience preferred.
• Home health or Hospice experience is a plus.
• Must have current Driver’s license and reliable transportation.

Responsibilities
