Trinity Health At Home – Grand Rapids (Michigan) seeks a dedicated Home Health Registered Nurse (RN) to deliver skilled nursing care in patients’ homes, developing personalized care plans and teaching patients and families to support recovery and independence.

You’ll practice at the top of your license with flexible autonomy, join a nurse-led team, and enjoy meaningful relationships with patients while contributing to high-quality home health care and career growth.

Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred
• Home health or Hospice experience is a plus
• Must have currentDriver’s license and reliable transportation.

Responsabilités
