Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes, develop care plans and educate families. You will work 1:1 with patients, supporting recovery and independence while staying compliant with standards.

Qualifications

• Graduate of an approved Nursing education program; BSN preferred.
• Current Registered Nurse licensure in Michigan.
• Minimum of one (1) year professional RN experience.
• Home health or Hospice experience is a plus.
• Strong computer and technology skills.

Responsibilities

• Deliver skilled nursing care in patients' homes and implement individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence with 1:1 patient relationships.
• Operate within top of license with flexibility and autonomy.
• Document and coordinate care using the EMR system.

MinimumQualifications

• Graduate of an approved Nursing education program, BSN preferred.

• Current Registered Nurse licensure in the State of Michigan.

• Must have a minimum of one (1) year professional registered nursing experience.

• Medical Surgical Experience preferred

• Home health or Hospice experience is a plus

• Strong computer and technology skillsrequired.

• Must havecurrentDriver’s license and reliable transportation.
