Job summary

Select Home Health is seeking a skilled and compassionate Registered Nurse to provide in-home nursing care in McCordsville, IN. You will assess patients, implement care plans, and document OASIS for multiple patients daily.

Educational requirements include RN licensure and at least 1 year of nursing experience; valid driver’s license and TB test are required. You will educate patients and caregivers, coordinate with physicians, and supervise LPNs/HHAs as needed.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license for the practicing state.
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver's license, reliable auto insurance, and personal transportation.
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).

Job Summary

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
