Select Home Health is seeking skilled and compassionate Registered Nurses to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.

You will perform patient assessments, implement plans of care, educate patients and caregivers, coordinate services, and document with OASIS. Indiana RN licensure and at least 1 year of nursing experience are required. Valid driver’s license and TB test must be current.

Competenze

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license for the state of Indiana.
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current up to date TB test.

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.
