Job summary

Ascension Saint Joseph in Chicago, IL is seeking a Night Shift Full-Time Registered Nurse to deliver comprehensive direct nursing care on a medical-surgical unit. You will execute physician orders, implement evidence-based care plans, monitor patient conditions, and document changes accurately.

You will coordinate across multidisciplinary teams, manage complex treatment plans, and lead discharge planning from admission through transition to optimize patient outcomes.

Qualifications

• BLS Provider within 30 days of hire or transfer (AHA/ARC accepted).
• Illinois RN licensure required prior to hire.
• Diploma from an accredited school/college of nursing or active professional licensure at time of hire.

• Serve as the primary clinical coordinator across multidisciplinary healthcare teams to streamline communication, manage complex treatment plans, and ensure well-coordinated, high-quality patient care.
• Proactively assess, manage, and coordinate comprehensive patient discharge planning needs from admission to transition, collaborating closely with case management and the healthcare team to optimize outcomes.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
