Job summary

Elite Care Management, Inc. in Sleepy Hollow, IL is seeking a dedicated registered nurse or licensed practical nurse for a part-time, one-on-one home health care role. You will care for a single patient per shift, assess needs, and implement the patient care plan.

Qualifications

• RN or LPN license valid in Illinois.
• Ability to care for one patient per shift.
• Current CPR certification.

Responsibilities

• Observe patient, record significant conditions, and notify supervisor or physician of changes.
• Adhere to nursing policies and procedures.
• Demonstrate strong communication, both spoken and written.
• Manage and direct patient care per the care plan and update as needed.
• Administer medications and treatments as prescribed and document effectiveness.
• Report changes in patient condition to physician or supervisor.

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.


• Demonstrate understanding organizational and nursing policies and procedures.


• Demonstrate strong communication skills, both spoken and written.


• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.


• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.


• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
