The role involves completing OASIS assessments (Start of Care, Recertification, Discharge, Resumption of Care), coordinating skilled visits, medication passes, and supervisory visits, and rotating on-call duties.

CHN is seeking a Full or Part Time RN case manager for Wayne County.

Case Managers are responsible for completing OASIS assessments (Start of Care, Recertification, Discharge, Resumption of Care) as well as Skilled Visits, Med Passes, and Supervisory Visits. On-Call rotation required.
