Pride Health is seeking a Registered Nurse - Home Health to support our client’s healthcare facility in Lexington, KY.

Schedule: Monday–Friday, 8:00 AM–4:30 PM. This local contract role covers Fayette, Jessamine, Garrard, Madison, Powell, Montgomery, and Clark counties with a pay rate of $53.00–$56.00/hour (W-2) plus mileage reimbursement.

Qualifikationen

• License: Current and unrestricted RN licensure in state of practice.
• Certification: Current CPR Certification.
• Years of experience: 1+ years in Home Health; Homecare Homebase (HCHB) is a plus.

• Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.
• Educate patients and caregivers on disease management, medication administration, infection prevention, medical equipment uses, and self-care techniques to improve health outcomes and promote independence.
• Maintain accurate clinical documentation, communicate changes in patient condition to the interdisciplinary healthcare team, and ensure compliance with agency policies, Medicare guidelines, and professional nursing standards.

Job Type: Local contract

Contract Length: 18+ Weeks (Possibility to Extend)

Pay Rate: $53.00 - $56.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.51/ mile

*Pay offered is based on experience,expertise, credentialing, and education.
