Pride Health is seeking an RN - Home Health/Case Manager to support our client's healthcare facility in Uniontown, Ohio.

This contract role covers Summit, Portage, and Medina Counties, with a pay range of $52-$54 per hour, dependent on experience and credentials. Responsibilities include coordinating patient care, assessing needs, developing plans, communicating with physicians, and ensuring timely documentation.

*Pay offered is based on experience, expertise, credentialing, and education.
