Job summary

Pride Health is seeking a Registered Nurse (RN) - Home Health for a local contract in Denver, CO. The role involves comprehensive home health nursing, SOC/POC management, and care-team coordination.

The ideal candidate has an active RN license, ADN or Nursing Diploma, 1–2 years of clinical nursing and at least 2 years in Home Health, plus OASIS and HCHB experience, CPR certification, and a valid driver's license. Pay range is $50–$54 per hour.

• Graduate of an accredited ADN program or Nursing Diploma program.
• Minimum 1–2 years of clinical nursing experience.
• Minimum 2 years of Home Health experience.
• OASIS assessment experience required.
• Homecare Homebase (HCHB) experience required.
• Current CPR Certification.
• Valid driver's license.

Responsibilities

• Perform comprehensive patient assessments in home health settings, including admissions, SOC, Recertifications, ROC, and Discharge visits.
• Develop, implement, evaluate, and update individualized Plans of Care (POC).
• Coordinate care with physicians, therapists, caregivers, referral sources, and interdisciplinary team members.
• Provide skilled nursing services including medication administration, wound care, disease management, IV therapy, catheter care, venipuncture, and patient education.
• Assess and manage pain, monitor progress, and identify changes in patient conditions.
• Educate patients and caregivers regarding medications, home safety, disease processes and self-care practices.
• Supervise Home Health Aides (HHA), LVNs/LPNs, and other staff as appropriate.
• Ensure timely and accurate clinical documentation utilizing Homecare Homebase (HCHB).

Schedule: 8 Hours Per Day, Monday to Friday, 08:00 AM to 05:00 PM

Location: Denver, CO,80221

Float / Coverage Area: Local Travel required

Pay Range: $50.00 to $54.00/hour*

Mileage Reimbursement Rate (if applicable): $0.51/ mile

*Pay offered is based on experience, expertise, credentialing, and education.
