Job summary

Pride Health is hiring a Registered Nurse (RN) - Home Health to support a healthcare facility in Lincoln, NE. The role is a full-time, local contract for 14 weeks with a workday from 9:00 AM to 5:00 PM, Monday through Friday, and local travel required.

The RN will perform SOC, Recertifications, ROC, and Discharge visits, develop and evaluate Plans of Care, provide wound care and IV therapy, and maintain documentation in HCHB while ensuring compliance with Medicare and payor requirements.

Qualifications

• Active State RN license required.
• Minimum 2+ years of Home Health experience.
• OASIS experience required.
• Homecare Homebase (HCHB) experience required.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• 1-2 years RN experience in a clinical care setting.

Responsibilities

• Complete comprehensive patient assessments, including SOC, Recertifications, ROC, and Discharge visits.
• Develop and evaluate individualized Plans of Care with physicians and interdisciplinary team.
• Provide skilled nursing services including wound care, venipuncture, IV care, catheter management, medication administration, and patient education.
• Maintain accurate clinical documentation utilizing HCHB and ensure Medicare/payor/agency compliance.
• Coordinate care with physicians, therapists, caregivers, and home health aides for continuity and quality.
• Participate in on-call rotations and interdisciplinary team meetings as required.

• Schedule: Full-Time | 8 Hours Per Day | Monday to Friday | 09:00 AM to 5:00 PM
• Location: Lincoln, NE, 68510
• Float / Coverage Area: Local Travel required
• Job Type: Local contract

• Pay Range: $48.00 to $51.00/hour*
• Mileage Reimbursement Rate (if applicable): $0.50/ mile

Duties

• Develop, implement, and evaluate individualized Plans of Care (POC) in collaboration with physicians and interdisciplinary team members.
• Provide skilled nursing services including wound care, venipuncture, IV care, catheter management, medication administration, disease management, and patient education.
• Maintain accurate clinical documentation utilizing Homecare Homebase (HCHB) and ensure compliance with Medicare, payer, and agency requirements.
• Coordinate care with physicians, therapists, caregivers, and home health aides to ensure continuity and quality of care.
• Participate in on-call rotations and interdisciplinary team meetings as required.

Requirements
