Job summary

UnitedHealth Group is seeking a Registered Nurse for Home Health in Dushore, PA. You will provide and direct nursing care, coordinating with physicians and other professionals to ensure continuity of care in patients' homes.

The role requires an active RN license in PA, 1+ year Home Health experience preferred, and a valid driver's license with reliable transportation. CPR certification is preferred or obtainable within 90 days of hire.

Qualifications

Responsibilities

• Develop and implement the plan of care in collaboration with physicians and health professionals.
• Supervise home health aides and LPNs/LVNs, and assign tasks.
• Perform supervisory visits as required.
• Deliver clinical services within scope and infection control standards.
• Complete OASIS assessments and reconcile medications.
• Document visits per policy and payer requirements, align with policy.

About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. This role is ideal for nurses who are passionate about delivering quality care in a home setting.

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education:

• Current and unrestricted RN licensure in state of practice

Experience:

• 1+ years of Home Health experience preferred

Licenses & Certifications:

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
