Job summary

UnitedHealth Group is seeking a Registered Nurse for home health in Gulf Shores, AL. You will plan and provide nursing care in patients' homes, coordinating with physicians and staff to ensure continuity of care and individualized treatment.

Responsibilities include plan-of-care development, supervision of home health aides and licensed nurses, OASIS assessments, and timely documentation. Requires RN licensure and CPR; driver’s license and vehicle insurance are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience preferred.
• Current Driver's License and vehicle insurance required.
• Current CPR Certification orability to complete within 90 days of hire preferred.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with agency staff and healthcare team members.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
