Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct patient care in patients\' homes, coordinating with physicians and professionals for continuity of care.

The role requires clinical leadership, supervision of aides and licensed nurses, and timely documentation. A valid RN license, CPR, and driver’s license are essential; 1+ year home health experience is preferred.

Qualifications

• RN licensure in the state of practice is required.
• 1+ year of home health experience preferred.
• Current CPR certification and valid driver's license are required.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise home health aides and licensed nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits for home health eligibility.
• Communicate timely information with agency staff.
• Assist in orienting new staff and serve as a preceptor to others.
• Participate in on-call and weekend rotations as needed.

About This Role

• Pay: $71,677 - $107,516 annual total cash target pay, $41.35 - $62.03 hourly rate, $34.46 - $51.69 per visit point

What You'll Do

• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

• Education: Current and unrestricted RN licensure in state of practice
• Experience: 1+ years of Home Health experience
• Licenses & Certifications: Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation; Current CPR Certification
