UnitedHealth Group is seeking a Registered Nurse for Home Health in Heber Springs, AR. You will provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to ensure continuity of care.

Responsibilities include developing care plans, supervising nurses and aides, performing supervisory visits, and completing OASIS assessments with precise medication reconciliation. A valid AR RN license and CPR are required or pursued promptly.

Qualifikationen

• Active RN license in Arkansas with no restrictions.
• At least 1 year of home health experience preferred.
• Current CPR certification or ability to obtain within 90 days.

About This Role

As the Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care.

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education:

• Current and unrestricted RN licensure in state of practice

Experience:

• 1+ years of Home Health experience preferred

Licenses & Certifications:

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
