Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care.

This full-time role in Hazleton, PA offers a $10,000 sign-on bonus for external hires and a total cash target pay range of $71,677 to $107,516. You will supervise aides, perform OASIS assessments, and participate in on-call rotations to meet patient needs.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current CPR Certification required.
• Current Driver's License with vehicle access or reliable transportation.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education

• Current and unrestricted RN licensure in state of practice

Experience

• 1+ years of Home Health experience

Licenses & Certifications
