Job summary

Qualifications

• Active RN licensure in the state of practice.
• Experience in home health is preferred.
• Current CPR certification or ability to obtain within 90 days.

Responsibilities

• Initiate, develop, implement, and revise the plan of care with physicians and team.
• Supervise care by home health aides and licensed nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services within scope and infection control standards.
• Complete comprehensive assessments (OASIS) and reconcile medications accurately.
• Document patient visits per policy and payer requirements.
• Make initial and comprehensive nursing evaluation visits for eligibility.
• Communicate timely with agency staff and physicians.
• Participate in orientation of new staff and act as a preceptor.
• Participate in on-call and weekend rotation as needed.

About This Role

As the Weekend Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care.

What You’ll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with agency staff and physicians.
• Assist in the orientation of new agency personnel and serve as a preceptor.
• Participate in on-call and weekend rotation as needed.

Requirements

• Education: Current and unrestricted RN licensure in state of practice
• Experience: 1+ years of Home Health experience preferred
• Licenses & Certifications: Current Driver's License and vehicle insurance required
• Current CPR Certification or ability to complete within 90 days of hire preferred
