Job summary

UnitedHealth Group in Berwick, PA is seeking a Registered Nurse in Home Health to deliver and coordinate patient care in home settings, collaborating with physicians and care teams.

You will supervise home health aides, perform comprehensive assessments, maintain documentation, ensure eligibility and medical necessity, and participate in on-call rotations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification required.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
• Adhere to and participate in the agency's utilization management model.

About This Role
