Job summary

UnitedHealth Group's Optum division is hiring a Registered Nurse in Home Health to deliver and direct nursing care in patients' homes under physician guidance, ensuring compliance with laws and policies. You will lead a care plan, supervise home health aides and LPNs, perform supervisory visits, and complete OASIS assessments with accurate medication reconciliation.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience preferred.
• Current Driver's License and access to a vehicle or reliable transportation.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with agency staff and other clinicians.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

About This Role

Licenses & Certifications

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
