Responsibilities

• Initiate and revise the care plan with the physician and care team.
• Supervise home health aides and licensed nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services per scope and infection control standards.
• Complete comprehensive patient assessments (OASIS) and reconcile medications.
• Document patient visits per policy and payer requirements.
• Assess home health eligibility through initial/comprehensive visits.
• Communicate timely with agency staff and other providers.
• Assist orientation of new staff and serve as a preceptor.
• Participate in on-call and weekend rotations as needed.

About This Role

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education:

• Current and unrestricted RN licensure in state of practice

Experience:

• 1+ years of Home Health experience preferred

Licenses & Certifications:

• Current Driver's License and vehicle insurance required
• Current CPR Certification or ability to complete within 90 days of hire preferred
