Job summary

Ascension is seeking a Registered Nurse to coordinate the overall interdisciplinary plan of care from admission to discharge. This role emphasizes patient-centered care, collaboration with the care transition team, and advocacy to remove barriers to ensure appropriate services and documentation.

The position requires Wisconsin RN licensure (or multi-state/Compact) and a Diploma in Nursing, with preferred BLS and CCMC Case Manager certification. Comprehensive benefits are provided.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• Preferred Credential(s):
  • BLS Provider. American Heart Association or American Red Cross accepted.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
