Job summary

Ascension in Milwaukee, WI is seeking a Case Management RN to architect care plans bridging hospital and home, advocate for patients, and coordinate multidisciplinary communication to ensure seamless health plan execution. The role requires a nursing license and experience in case management.

Minimum requirements include a nursing diploma and Wisconsin RN licensure; BLS and CCMC case manager credentials are preferred. This is a day shift, PRN availability in the Milwaukee facility.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• BLS Provider credentialed from the American Heart Association (AHA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education

• Diploma from an accredited school/college of nursing required.
