Job summary

Ascension in Appleton, WI, is seeking a Case Management RN to architect comprehensive care plans bridging hospital and home and advocate for patients to remove barriers to care. You will coordinate clinical communication across a multidisciplinary team to ensure seamless health plan execution and maintain documentation integrity reflecting patient journey complexity.

Minimum requirements include RN licensure in Wisconsin, BLS preferred, and CCMC preferred.

Qualifications

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• BLS Provider credentialed from the American Heart Association (AHA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

• Champion documentation integrity by ensuring medical records reflect the necessity of services and the complexity of the patient journey.
