The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Dover Hospice (Homestead), New Philadelphia, OH. You will coordinate patient care, manage care plans, and work with physicians, caregivers, and the interdisciplinary team to ensure comfort and dignity at end of life.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management and end-of-life care, providing compassionate support.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
