Job summary

The Care Team Hospice in Canton, OH is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop plans of care, and collaborate with the interdisciplinary team to ensure comfort and dignity at end of life.

Qualified candidates hold an RN license with hospice experience, strong assessment and communication skills, and familiarity with Homecare Homebase (HCHB). This role includes day hours, on-call opportunities, and a supportive work culture with competitive benefits.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
