Job summary

The Care Team Hospice is seeking a compassionate and patient-focused RN Case Manager to join our team. This nurse will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and our interdisciplinary team to ensure comfort and quality of life.

Qualifications

• Active RN license required.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours/on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients and families on symptom management and end-of-life care.

• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
