Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Highland Heights/Greater Cleveland. You will coordinate patient care, develop plans of care, and work with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life.

Mon-Fri, 8:00 AM–4:30 PM with on-call opportunities and potential additional compensation. We offer a supportive, growth-focused environment with comprehensive benefits and opportunities for advancement.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and interventions with accurate documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours situations, to ensure quality patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and outcomes.
• Educate patients and families on symptom management and end-of-life care.

Schedule & Work-Life Balance

• Monday-Friday, 8:00 AM - 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only

Key Responsibilities:

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
