Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team and deliver exceptional end-of-life care in Holmes/Tuscarawas Counties.

You will coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and interventions with timely documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and the interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients and families on symptom management, disease progression, and end-of-life care.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
