Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Akron, OH. This role coordinates patient care, manages individualized plans of care, and collaborates with physicians and families to ensure comfort and quality of life.

We offer on-site care in Northern Summit and Portage Counties with a Monday–Friday schedule, 8:00 AM to 4:30 PM, plus on-call opportunities for added compensation. Join us to deliver exceptional end-of-life care where patients call home.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and care plans.
• Complete scheduled visits, admissions, and interventions with timely documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours on-call when required.
• Collaborate with physicians, caregivers, and the interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.

Schedule & Work-Life Balance

Key Responsibilities:

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
